Five charges sit outside the headline PEPM often enough to break a budget built on that number alone, and most are hard to renegotiate after signature.
834 EDI carrier feeds
An electronic eligibility feed to each carrier is priced separately and is frequently the reason a quote moves. Employee Navigator bills agencies $0.45 per enrolled employee per month based on the highest eligible feed per group, requires its Enhanced Plus license or higher, and waives the fee on its Platinum tier.[2][1] It warns that setup takes longer than its other integrations because a file has to be built and tested for each company individually, so confirm your broker's tier before assuming feeds are available.[2]
ACA reporting and 1095-C filing
ACA forms are priced per form per year rather than per employee per month, and Ease charges $6 per employee per form per year for 1094-B/C and 1095-B/C generation.[3] Filing bureaus are cheaper at volume: TaxBandits charges $2.99 per form for the first 49 falling to $1.99 at 501 to 1,000, plus $1.50 for direct state filing and $2.50 for postal mailing.[8] Sovos ACAMs charges $3.49 per form to print, mail and e-file at 1 to 10 forms, $1.29 at 501 to 1,000, and $0.59 above 10,001, with a $99 annual setup fee and state filing at $99 per state plus $1.49 per record.[9]
COBRA administration
COBRA is quoted on its own schedule and is rarely inside the ben admin PEPM. Professional Benefit Services publishes a 2026 schedule of $1.00 per active employee per month with a $50 monthly minimum, setup from $300 at 20 to 200 employees up to $1,000 above 800, and annual renewal of $200 to $700.[6] Flat annual pricing also exists, with COBRAadminOnline listing comprehensive service at $799 under 100 employees, $1,799 for 100 to 499, and $3,499 above 500.[7] Both bill on employee counts rather than per qualified beneficiary, so treat any per beneficiary quote as a different structure and rerun it on your own numbers.
Implementation and the annual rebuild
Setup is quoted once and open enrollment reconfiguration is quoted every plan year. Small and midsize builds run $1,500 to $5,000, while enterprise implementations run $25,000 to $50,000 standard and more with heavy carrier counts.[11][12] Ask what a plan change in year two costs, because that is where a low first year quote is usually recovered.
Payroll integration
Ease says payroll integration costs are partner specific and typically range from $0 to $1.50 PEPM.[3] Employee Navigator provides payroll integrations at no additional cost to brokers while noting that the client's payroll provider may charge them setup or integration fees.[2]
